Improve your health and well-being with HealthMeasures – an online health management tool provided to members at no extra cost.

You will have access to online dietary advice, exercise programs and health coaching. You can also create your very own well-being plan so you can choose the areas you would like to focus on to improve your health and well-being.

Managing your health and well-being online could not be easier. You can access your favourite resources wherever you are with the Well-being Plus iPhone and Android apps. You can download the apps in either of the following ways:

Teachers Health Fund members will be required to enter the code HM when downloading the apps. The mobile apps will prompt you when this is required.

Take the Well-being Assessment

The Well-being Assessment is a confidential questionnaire that assesses your lifestyle and overall health to help shape your very own well-being plan. The assessment takes into consideration that you may be more ready to start on some areas than others and can help you begin with small, manageable steps to get you on the right track. Take the assessment now.

Do you have a chronic condition?

If it is identified that you have a chronic condition the HealthMeasures program can provide you with care support services including:

  • Telephone support from registered nurses to help improve your health condition
  • A registered nurse care manager will help you draw up an online health action plan, which can then be monitored for progress.
  • Online access to approved health coaches for guidance on your diet, physical activity and lifestyle.
  • SMS or email reminders for any preventative and health screening tests you might need.
  • Reminder calls prior to your GP appointments.
  • Advice on local community services to help you with the maintenance of your condition.

All Australian residents, who are entitled to Medicare, are able to claim an income tested rebate on their private health insurance; would you like to apply your rebate?

For individuals earning between $88,001 - $102,000 (or $176,001 - $204,000 for families*).
*For families with children, the thresholds are increased by $1,500 for each child after the first.

How rebate tiers workX

Most Australians with private health insurance currently receive a rebate from the Australian Government to help cover the cost of their premiums. The private health insurance rebate is income tested. The table below details the different rebate amounts and Medicare Levy Surcharge levels.

The rebate applies to hospital, general treatment and ambulance policies. It does not apply to overseas visitors cover. The rebate levels applicable for 1 July 2013 to 30 June 2014 are:
